Purpose of the role
As a Support Worker in our supported living service, you will provide person-centred care and support to adults living in their own tenancies. The aim is simple: help each person we support to live the life they want, with the right balance of help and independence. You will build trust over time with the people on your caseload, deliver their care plans accurately and safely, and play a genuine part in their quality of life.
You will work both one-to-one and as part of a small, consistent staff team.
Key responsibilities
Person-centred support
- Deliver care and support in line with each person's care plan, risk assessment and positive behaviour support plan where one is in place.
- Support the people you work with to make their own choices and decisions, in line with the Mental Capacity Act 2005.
- Build respectful, consistent relationships with the people you support and the people who are important to them.
- Promote independence at every opportunity — doing things with, not for.
Daily living
- Support with household tasks: cooking, cleaning, laundry, shopping, budgeting and bill management, at the level agreed in the care plan.
- Support with healthy eating, meal planning and food preparation.
- Support with community access: appointments, leisure, social activities, employment or education where relevant.
Medication
- Administer or prompt medication in line with MAR charts and iCare's medication policy, once trained and signed off as competent.
- Record medication accurately and promptly. Report any refusals, errors, side effects or near-misses without delay.
Health, safety and risk
- Follow each person's risk assessment and care plan, including any positive risk-taking plans agreed with the Registered Manager.
- Use prescribed equipment correctly (hoists, slide sheets and other moving and handling aids) in line with training.
- Maintain a clean, safe and well-ordered environment in line with infection prevention and control procedures.
- Complete environmental, fire safety and property checks as required.
Safeguarding
- Act as a first line of safeguarding: recognise and respond to signs of abuse, neglect or harm.
- Report safeguarding concerns to the Registered Manager or on-call manager without delay.
- Follow iCare's safeguarding policy and, where relevant, local authority safeguarding protocols.
Communication and records
- Record all care delivered, observations, incidents and communications accurately, factually and in a timely manner, on the CareFree / iCare electronic care record.
- Participate in handovers at the start and end of every shift.
- Communicate clearly and respectfully with colleagues, managers, families (with consent), other professionals and the people we support.
Positive behaviour and de-escalation
- Where a person's plan includes PBS or behaviour support strategies, follow these consistently and contribute to their review.
- Use trauma-informed, de-escalation and low-arousal approaches in line with training.
Working with the wider team
- Attend team meetings, supervision, appraisal and reflective practice sessions.
- Work alongside district nurses, GPs, CMHT, CPNs, social workers, OTs, physiotherapists and other professionals involved in each person's care.
- Participate in care plan reviews and contribute your observations.
- Support new colleagues during their induction and shadow shifts.
Out-of-hours and night cover
- Work shifts as allocated by the rota, including early mornings, evenings, weekends and bank holidays.
- Carry out sleep-in or waking-night duties where these are required by the service need, notified in advance on the rota.
Duty of candour and quality
- Act openly and honestly when things go wrong, in line with the duty of candour under CQC Regulation 20.
- Contribute to continuous improvement of the service, including through feedback, audits and learning from incidents.
