Community Support & Prevention Worker
Hours: 37.5 hours weekly – Mon-Fri
Salary: £13.09 per hour
Friendship at Home Friendship at Home is a registered charity dedicated to supporting older people within North-East Lincolnshire. We believe no older person should experience loneliness and that everyone should have opportunities to engage with their community. For 20 years, we have remained true to our founding principles of maximising independence, reducing loneliness and isolation, now serving over 3,000 members annually. We provide an established hospital discharge support service, wellbeing service, befriending, a wide range of social and exercise activities, and offer information and advice on numerous issues. We are a small, dedicated staff team of 18, strongly supported by over 140 passionate volunteers.
The Community Support Worker is a vital frontline staff role responsible for delivering focused, flexible, short-term (up to 4 weeks) practical, emotional, and re-ablement support to vulnerable adults in NEL. The post-holder ensures smooth service delivery from referral response to service discharge, providing ongoing assessment, developing support plans, and acting as the primary link between the service user, the volunteer team, and external health/social care professionals (including SPA).
Person Specification:
Knowledge and Skills
· Ability to assess service users’ needs and complete/follow a short‑term support plan.
· Working knowledge of health and social care services (NHS, Social Care, VCSE).
· Excellent interpersonal and communication skills, with the ability to liaise effectively with multi‑disciplinary teams.
· IT literate (Microsoft 365, mobile apps) with strong recording and reporting skills.
· Ability to work as part of a team and manage own workload, prioritising competing demands.
Experience
· Experience supporting service users who may be frail, cognitively impaired, or recovering from a hospital stay or injury.
· Experience delivering high‑quality, time‑limited services to the public.
· Experience contributing effectively within a multi‑disciplinary team environment.
· Experience carrying out risk assessments related to home safety and service user wellbeing.
Additional Requirements
· Criminal Records Check: Enhanced DBS (Adult Workforce) required.
· Driving: Full driving licence and access to own vehicle for travel and transporting/accompanying service users (mileage reimbursed). Appropriate insurance required.
· Flexibility: Willingness to work flexibly around service needs, including occasional evenings/weekends for events if required.
· Compliance: Adherence to FaH policies, including Lone Working, Manual Handling, and Infection Control.
