Working across supported housing and community outreach services, you'll support people to build independence, improve wellbeing and access the services they need.

Day-to-Day Duties:
- Provide practical and emotional support to individuals with a range of complex needs.
- Build positive, professional relationships that promote independence and recovery.
- Deliver person-centred, strengths-based and trauma-informed support.
- Offer advice, guidance and signposting to appropriate services and community resources.
- Maintain accurate and up-to-date client records and support plans.
- Work closely with housing providers, healthcare professionals and other agencies to coordinate effective support.
- Safeguard vulnerable adults and follow relevant safeguarding procedures.
- Travel between services across the South West, supporting teams where additional cover is required.
- Adapt to changing priorities and work flexibly to meet service needs.
About You
- We're looking for someone who is compassionate, adaptable and confident working with people facing complex challenges.
- You will ideally have:
- Experience supporting vulnerable adults within housing, homelessness, mental health, substance misuse or related services.
- An understanding of trauma-informed, strengths-based approaches.
- Excellent communication and relationship-building skills.
- Strong organisational skills with the ability to manage changing priorities.
- Confidence working independently as well as part of a wider team.
- A full UK driving licence and access to your own vehicle (business insurance required).
- The flexibility to travel regularly across Bristol, Bath & North East Somerset, Wiltshire, Somerset and North Somerset, with occasional early starts where required.
What's on Offer
- £28,655 annual salary.
- Permanent, full-time position.
- Flexible working pattern with the option of a 4-day working week.
- Generous annual leave entitlement that increases with length of service.
- Competitive pension scheme.
- Health and wellbeing support, including an Employee Assistance Programme.
- Ongoing training, reflective practice and career development opportunities.
- Retail discount scheme and Blue Light Card eligibility.
- A supportive, inclusive workplace that values diversity, wellbeing and professional development.
