Manor View Practice is a leading large GP practice dedicated to delivering comprehensive and compassionate care to individuals with diverse needs. We are seeking a compassionate Care Coordinator to join our team at Manor View Practice. The Care Coordinator supports patients—especially those with long-term conditions, complex needs, or frailty—by organising and managing their care across different services.
The successful candidate will be responsible for coordinating and facilitating care services across multi-disciplinary teams to ensure the highest quality of care and support for these patient cohorts.
Key Responsibilities
- Care planning
- Help coordinate and maintain personalised care plans for learning disabilities, ASD and adult ADHD patents.
- Work with GPs, nurses, and wider MDT (multidisciplinary team)
- Patient support
- Act as a key contact for patients and carers
- Help patients understand their conditions and treatment plans
- Coordinating services
- Arrange annual reviews, referrals and follow-ups
- Liaise with hospitals, community services, social care, other external healthcare professionals and voluntary adult services
- Admin and documentation
- Maintain accurate patient records
- Input care information into various IT systems (eg. EMIS, Accurx,)
- Supporting vulnerable groups
- Patients with multiple conditions (e.g. diabetes + COPD)
- Elderly or housebound patients including those in care homes
- Patients at risk of hospital admission
Person Specification:
· Excellent communication skills
· Good oral and written English language skills
· GCSEs including Maths and English level 4 or above
· A driven, self-motivated, and passionate person who believes in providing holistic care and moulds well with the ethos of Manor View Practice
· A quick learner who isn’t afraid of getting stuck in and figuring things out on their own but is also willing to ask for help when needed.
· You must be a team player who enjoys working with colleagues to problem solve
· IT skills, typing, previous experience of using Microsoft Office suite including Excel and Teams
· Car driver, hold a current driving licence and prepared to travel to MVP sites
Desirable Experience:
· Previous experience in NHS primary care
· Has experience working with the EMIS IT system.
Pay: From £13.06 per hour
