As a Social Worker with our Hospital Discharge Team you can make your own powerful contribution to Westminster’s success. Based in St Mary’s Hospital and operating across the Borough of Hammersmith and Fulham, the Royal Borough of Kensington and Chelsea and Westminster City Council, you’ll work closely with health colleagues within an acute setting, and be an active member of a ward multi-disciplinary team. Undertaking assessments of need and developing support plans that help patients with their discharge from hospital, you’ll also undertake safeguarding investigations and Mental Capacity Act assessments, and work with carers and other professionals to minimise and manage risk while helping each patient to maintain their chosen lifestyle.
Offering advice and information to your colleagues and other stakeholders on the role of the social worker and the service we provide, you’ll join our urgent response and duty system and participate in developments to our service. Committed to ensuring that your professional registration and DBS are kept up to date, we’ll also expect you to be responsible for your own CPD, and participate in appraisals, audits, evaluations, reflective practice and team meetings.
It’s important that you work in line with all our policies and procedures including health and safety, data and case recording, supervision, data protection and information governance, and you should be ready to inform your manager of any significant events relating to the service. Dedicated to undertaking best interest and practice teacher training as part of your development, you’ll also take part in Trusted Assessor training – this will enable you to prescribe simple equipment and assistive technology that helps to maximise your customers’ independence. With this in mind, you’ll adopt an inter-disciplinary approach to your work and collaborate with all our partners in order to achieve best outcomes.
You should be aware that this role involves working five days a week including Saturdays, Sundays and Bank Holidays. You’ll work as part of a team for three days a week, and although you’ll be the only Social Worker on site over weekend and holiday periods, a Duty Manager will be available remotely to provide support.
