About Us: Hannah-Grace Care and Support Ltd is a provider of support services dedicated to supporting young people aged 16-18. Hannah-Grace supports young people in obtaining independent living skills whilst being supported by experienced staff in an environment identical to most family homes.
Role Overview:
As a Support Worker, you will play a role in creating a safe and secure environment where young people can feel protected and supported. Your duties will involve implementing robust safeguarding practices, including risk assessments, incident reporting, and adherence to safeguarding policies and procedures.
You will be responsible for providing direct support, facilitating key work sessions, and promoting positive relationships built on trust and respect.
Key Responsibilities:- Provide high-quality support to young people aged 16-18, prioritising their safety and well-being at all times- Maintain a vigilant and proactive approach to safeguarding, identifying, and escalating any concerns promptly and in line with safeguarding policies and procedures- Conduct key work sessions with our young people with a focus on their individual needs- Assist young people with activities of daily living, including cooking, household tasks, attending appointments, and budgeting while promoting independence and self-care skills- Facilitate and participate in activities that benefit our young people- Collaborate effectively with colleagues, and families to ensure a coherent approach to safeguarding and support

Practical and safeguarding requirements
- Basic literacy and numeracy sufficient to read care plans and record notes clearly.
- Willingness to undertake mandatory training (e.g. care certificate, moving and handling, safeguarding).
- Right to work in the UK and ability to pass an enhanced background (DBS) check.
Ideal
Previous experience working in a similar role within residential support or a related field- An understanding of safeguarding principles and legislation, with a commitment to upholding the highest standards of safeguarding practice- Excellent communication and interpersonal skills- Flexible to work day and night shifts as required, including weekends and bank holidays.
Qualifications: NVQ Level 3 or working towards.
