Job Summary
We are seeking a dedicated and organised Care Co-ordinator to oversee and manage care services within a care home specialising in senior care. The successful candidate will play a pivotal role in ensuring the delivery of high-quality care, supervising staff, and maintaining effective communication with residents, families, and healthcare professionals. Responsibilities
- Coordinate daily care activities to meet individual resident needs, ensuring personalised care plans are followed accurately.
- Supervise and support care staff, fostering a positive team environment and promoting best practices in senior care.
- Develop, review, and update care plans in collaboration with residents, families, and healthcare professionals.
- Organise staff rotas and allocate resources efficiently to ensure consistent and high-quality care delivery.
- Maintain clear and professional communication with residents’ families regarding care updates and any concerns.
- Ensure compliance with health and safety regulations and company policies across all aspects of care provision.
- Oversee documentation processes using IT systems to record resident information, incidents, and care activities accurately.
- Assist with the mobilisation of residents when necessary, including driving responsibilities if required.
Requirements
- Proven experience supervising staff within a homecare or senior care environment.
- Strong leadership skills with the ability to motivate and guide a team effectively.
- Excellent communication skills, both written and verbal, to liaise confidently with residents, families, and healthcare professionals.
- Competent IT skills for managing electronic care plans and documentation systems.
- Valid driving licence is desirable for transportation duties or attending external meetings.
- Knowledge of developing and implementing personalised care plans tailored to individual needs.
- Experience working within a children, young people or elderly care setting is highly advantageous.

Pay: £24,000.00-£30,000.00 per year
