About the Role
We are seeking an experienced and motivated Advanced Clinical Practitioner (ACP) or Advanced Nurse Practitioner (ANP) to join our friendly, supportive and forward-thinking primary care team.
This is an exciting opportunity to work within a well-established GP practice where you will play a key role in delivering high-quality, patient-centred care. The successful candidate will work autonomously whilst being fully supported by an experienced multidisciplinary team.
We are particularly interested in candidates with previous General Practice experience who are confident in managing undifferentiated presentations and making independent clinical decisions.
Main Responsibilities
The successful candidate will:
- Independently assess, diagnose, treat and manage patients presenting with acute and chronic conditions.
- Undertake comprehensive clinical assessments, formulate differential diagnoses and develop evidence-based management plans.
- Prescribe safely and appropriately within their scope of practice.
- Request, interpret and act upon investigations including blood tests and diagnostic imaging.
- Manage same-day urgent care appointments and minor illness clinics.
- Provide health promotion, preventative care and patient education.
- Refer patients appropriately to community, secondary care and PCN services.
- Work collaboratively within a multidisciplinary team including GPs, Practice Nurses, Pharmacists, First Contact Practitioners and other healthcare professionals.
- Contribute to clinical governance, quality improvement and service development initiatives.
Essential Criteria
- Registered healthcare professional with appropriate regulatory body registration (NMC, HCPC or equivalent).
- Master's Degree or Postgraduate Diploma in Advanced Clinical Practice.
- Independent Prescriber qualification.
- Evidence of advanced clinical assessment and diagnostic skills.
- Excellent communication and consultation skills.
- Ability to work autonomously and manage clinical risk effectively.
- Previous experience working in General Practice or Primary Care.
Desirable Criteria
- Experience using SystmOne.
- Minor illness and urgent care experience.
- Experience supervising or mentoring colleagues and trainees.

Working Hours
Flexible hours available between:
Monday to Friday: 08:00 – 18:30

Please note: Due to the autonomous nature of this role, applicants should ideally have previous experience working within a General Practice or Primary Care setting. Unfortunately, we are unable to provide training placements for newly qualified ACPs/ANPs without relevant primary care experience.
