Tieve Tara Medical Centre which is part of Spectrum Community Health CIC, are looking for a Practice Nurse to join our friendly nursing team. We are a busy surgery with approximately 5,500 patients so an adaptable approach is needed.
We are seeking to appoint an individual with a caring nature who will provide and maintain a professional manner and a high standard of nursing care for our patients.
This is a fantastic opportunity for a Practice Nurses with good communication skills, and a commitment to working as part of a strong and effective team.
Main duties of the job Ideally the successful applicant will be qualified to undertake chronic disease management (Diabetes, and/or COPD and Asthma), travel health, women's health and all general treatment room tasks. We would be supportive of any training requirements.
Applicants must be a fully qualified Registered General Nurse (RGN) and have a valid NMC number.

The flexibility to be able to cover sick/annual leave at times if required.

Management of Chronic Disease:
Diagnosis, monitoring and development of individual management plans: agreeing these as appropriate with the patient and other health professionals.
Therapeutic Monitoring:
Checking compliance with the adherence to appropriate treatments using a holistic patient-centred approach.
Recognising abnormalities
Identifying the impact of treatment and implementing or recommending changes as appropriate.
Health Promotion:
Provide patients with comprehensive information about potential health risks and chronic disease, using education, discussion and appropriate literature, to promote understanding of lifestyle issues, in particular smoking cessation, healthy eating and regular exercise.
Patient Health Checks:
Working with patients to develop a management plan where health problems or potential health problems are identified
Health screening:
Provide in depth monitoring and advice as appropriate and work with the patient in deciding on management plans.
Travel Health:
Undertaking a pre-travel risk assessment using the information the patient has provided on their travel form prior to their appointment.
Identifying problems
Administering travel vaccinations as appropriate, using either a valid Patient Group
Directive or Patient Specific Directive according to the current guidelines.
Providing patients with evidence based up to date information with regard to any given destination and advice associated with any potential health risks.
Family Planning:
Contribute to the provision of a comprehensive family planning service including emergency contraception and sexual health advice and screening.
Cytology:
Undertake cervical smears following the Practice and national guidelines.
Keep a personal record of smears taken for audit and where necessary follow up.
Pathological Specimens and Investigatory Procedures:
Following appropriate training, undertake the collection, storage and dispatch of pathological specimens including intravenous blood samples, swabs, smears etc and ensure the cleanliness of any equipment so used. Pathology Results: Address pathology results at the request of the GP’s acting as directed by the documentation attached to the request.
Discuss blood results with patients in the context of relevant LTC e.g.HbA1c Address cytology results, documenting these results in the patient notes and in the individual smear-taker logbook to allow personal audit of results and follow up of abnormal smears.
