Based within Torridge Health PCN in Longbridge House, working across the 6 practices in Torridge and in the community.
Job Purpose
The Dementia Support Worker will work as part of the Primary Care Network (PCN) to provide personalised, practical, and emotional support to people living with dementia, their carers, and families. They will act as a key point of contact, helping individuals to navigate health, social care, and voluntary sector services, promoting independence, wellbeing, and quality of life.
Key Responsibilities
- Patient & Carer Support
- Provide one-to-one support for people with dementia and their carers, offering information, advice, and signposting.
- Undertake holistic needs assessments and develop personalised support plans.
- Offer emotional support and practical guidance throughout the dementia journey, from diagnosis through to advanced stages.
- Care Coordination
- Work closely with GPs, nurses, social prescribers, care coordinators, and community organisations to ensure joined-up care.
- Support patients and carers to access statutory services, benefits, social care packages, and community resources.
- Help with navigating appointments, referrals, and communication across health and social care.
- Community Engagement
- Develop links with local dementia support groups, charities, and community services.
- Support awareness-raising and dementia-friendly initiatives within the PCN.
- Advocacy & Empowerment
- Promote independence, dignity, and choice for people living with dementia.
- Empower carers with coping strategies, peer support, and respite opportunities.
- Monitoring & Reporting
- Maintain accurate records of patient interactions in line with PCN and GDPR requirements.
- Collect data to evidence outcomes and support PCN reporting requirements.
- Contribute to audits, service evaluation, and quality improvement initiatives.
Person Specification
Essential:
- Experience of working with people with dementia or vulnerable adults.
- Knowledge of dementia, memory problems, and the challenges faced by carers.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a multi-disciplinary team.
- Organisational skills, with ability to manage a caseload.
- Empathy, patience, and resilience.
Desirable:
- Training in dementia care (e.g., Dementia Friends, Dementia Champion, NVQ/Health & Social Care qualifications).
- Experience of working in health, social care, or voluntary sector settings.
- Knowledge of local services, benefits, and support pathways.
