- Hours: 40 per week
- Monday to Friday 9am - 5pm (Saturdays on a rota basis, with day off in the week)

Job Summary:
As Pathway Co-ordinator (Autism360) you will play a vital role in supporting patients throughout the assessment journey. This position ensures that patients receive clear guidance, timely information and support, helping them understand each stage of the pathway. The role is responsible for administrative processes, coordinating communication between patients and clinicians, maintaining high‑quality documentation, whilst ensuring the overall smooth running of the autism pathway.
Essential Duties and Responsibilities:
- Draft letters and formal documentation for GPs, clinicians and patients, ensuring accuracy and timely delivery in line with KPIs
- Work collaboratively with clinicians to maintain robust documentation and ensure smooth progression for each patient
- Manage and respond to inbound enquiries across various communication channels, including telephone and email and make outbound calls
- Provide supportive information to help individuals navigate their assessment processes.
- Support with sharing relevant documentation and information to help patients record and understand their clinical progress
- Maintain consistent communication with clinical teams to ensure effective coordination of pathway activity
- Provide reliable administrative support that enables clinicians to focus on delivering outstanding patient care
- Upload SCR (Summary Care Record) documents accurately and in line with governance requirements
- Work agilely across the front office functions to ensure smooth delivery of operations.
- Review onboarding information for new patients to ensure accuracy, completeness, and readiness for clinical review (e.g., approving valid ID)
- Be part of a team that delivers the Health Division culture and align with values.
Qualifications and Experience:
- Strong understanding of administrative processes within healthcare, social care or a similar setting
- Knowledge of patient pathways or experience supporting individuals through clinical settings
- Understanding of confidentiality, data protection, and governance requirements, including safe handling of sensitive information (e.g., SCR documentation)
- Demonstrated experience in a busy administrative or coordination role
- Experience drafting professional letters, reports, or formal correspondence
- Experience handling patient, client, or customer enquiries across multiple communication channels (e.g., telephone, email)
- Experience working alongside clinical or multidisciplinary teams (MDTs)
- Positive outlook and an ability to work well under pressure
- Organisation skills and an ability to manage own workload
- Competent and confident with computers but not limited to MS Office, Outlook, Word and Excel.
- An ability to work on own initiative yet be a key part of a team
