We are seeking 1 dedicated and compassionate Receptionist/Admin to join our team. This role is ideal for a school leaver with an interest in health and social care, looking to take their first step into a career in primary care.
This position would suit a school leaver not enrolled or looking to enrol at a university.
As a Trainee Care Navigator, you will play a crucial role in assisting patients to navigate the healthcare system , ensuring they access the appropriate services and support tailored to their needs. You will develop key organisational and communication skills while working in a supportive office environment.
- Guide and support patients through their healthcare journey by providing clear information and signposting to appropriate services.
- Work closely with healthcare professionals to ensure patients receive timely and effective care.
- Maintain accurate records of patient interactions and service referrals.
- Assist with administrative tasks related to patient care coordination.
- Develop strong organisational skills and contribute to a positive team environment.
- Learn and adhere to data protection and confidentiality standards.

Duties
- Serve as the primary point of contact for patients, providing guidance and support throughout their healthcare experience.
- Assess patient needs and coordinate care plans tailored to individual requirements.
- Facilitate communication between patients, healthcare providers, and community resources to ensure seamless care transitions.
- Maintain accurate records of patient interactions and care plans in compliance with privacy regulations.
- Educate patients about available services, resources, and treatment options to empower informed decision-making.
- Collaborate with multidisciplinary teams to enhance patient care and address any barriers to access.
- Monitor patient progress and follow up as necessary to ensure adherence to care plans.
Experience
- Strong organisational skills with the ability to manage multiple tasks effectively in an office environment.
- Excellent communication skills, both verbal and written, with a focus on empathetic patient engagement.
- IT literate
- A commitment to continuous professional development and staying informed about healthcare trends and resources.

We are looking for x1 candidate FULL TIME DAYS/ HOURS TO BE DISCUSSED.
30 Hours per week
Hourly rate is age dependent £10.85-12.71 PH
