Job Summary
Grace Consulting is looking for a compassionate and motivated Care Adviser, ideally a registered nurse, to join our friendly team.
This is an opportunity to work in a rewarding role where your expertise and empathy will genuinely improve people's lives during some of their most challenging times, by providing personalised guidance on care options, funding, and support services. Through telephone, video consultations and care advice visits, you will help clients understand their options, access appropriate support and navigate health and social care systems.
You will:
- Conduct consultations with clients and family members.
- Provide information and advice on care options, funding and statutory services.
- Research suitable care providers and support services.
- Prepare detailed advice summaries and follow-up correspondence.
- Liaise with health, social care and other professionals where appropriate.
- Maintain accurate records and case notes.
- Work closely with colleagues to ensure excellent client outcomes
We're looking for someone who has:
- Experience within health, social care, care management, care navigation or a related field.
- A strong understanding of the UK health and social care system.
- Excellent communication and listening skills.
- The ability to build rapport quickly with clients and families.
- Strong research and report-writing skills.
- Good organisational abilities and attention to detail.
- Confidence using Microsoft Office and case management systems.
- Is a car driver

Pay: £40,000.00-£41,260.00 per year
