About the Role
Oakhaven Hospice is seeking a compassionate and experienced Clinical Nurse Specialist to join our palliative care team on a Bank basis. This is a flexible role, providing cover on an as-needed basis to support service demands, annual leave, and periods of increased activity across the Community Clinical Nurse Specialist service. Providing expert care and support to patients with advanced, life-limiting illnesses and their families, working closely with hospice teams, community health professionals, and hospital palliative care services.
key responsibilities will include:
Undertake holistic palliative nursing assessments and deliver hands-on care in community settings.
- Assess, plan, implement, and evaluate person-centred care plans in partnership with patients and families.
- Provide timely and responsive care, including urgent nursing interventions when required.
- Support patients and families with emotional, psychosocial, and spiritual needs, in person and via phone consultations.
- Promote patient choice, including preferred place of care and death.
- Act as a clinical resource and educator for health and social care professionals.
- Collaborate effectively with the multidisciplinary team.
- Provide clinical support and advice to the Palliative Home Care Team.
- Maintain accurate documentation, participate in audits, and contribute to quality improvement initiatives.
- Work flexibly across the service to meet patient and team needs.
You will have:
Current NMC Part 1 or Part 2 registration.
- Proven experience in palliative care, end-of-life care, or oncology, including caring for patients and families facing terminal illness.
- Community nursing experience, delivering patient-focused care in home or community settings.
- UK driving license with at least 3 years’ driving experience (essential for community visits).
- Excellent communication and interpersonal skills, able to support patients, families, and colleagues with empathy and clarity.
- Strong organisation, IT literacy, and problem-solving skills, with the ability to think creatively in complex situations.
- A compassionate, professional, and responsible approach to nursing practice.
