We are looking for a highly motivated clinical pharmacist to join the team providing pharmacy service s to the Rheumatology and Dermatology Clinical Teams. Joining an established team, you will play a key part in providing a first class, patient focussed service to patients across Cardiff and Vale University Health Board (CAVUHB) . Your knowledge, skills, and experience will further enhance the excellent service we provide.
Based at the University Hospital of Wales, the post holder will have the opportunity to provide clinical pharmacy services, run pharmacist-led clinics and manage complex drug regimes for patients of the highly specialised Rheumatology & Dermatology services. They will play an integral role in directing these services as part of the MDT, ensuring that a sustainable, high quality pharmacy service is provided for the users of these services.
The post holder will be supported in developing their advanced practice by our experienced team of pharmacists and the excellent team of
professionals working in both multidisciplinary teams. We will support your leadership development by providing access to recognised training and other structured opportunities to challenge and enable you to enhance your clinical role.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
Working closely with the MDTs, you will develop your role as a specialist prescribing pharmacist, running clinics to manage complex drug regimes, and deprescribing. You will be responsible for ensuring patients can access medicines in a safe and timely manner and arrange safe and timely discharges by liaising with community teams and primary care colleagues.
You will be a member of the Medicine pharmacy team, which has a proven track record for developing pharmacist roles. The team is also responsible for providing clinical pharmacy services to the wider Medicine wards at UHW.
As part of the pharmacy team, you will lead by example and be responsible for training and development of less experienced team members. Your professionalism, excellent inter-personal skills and enthusiasm, will equip you to be a valued contributor to our service and to develop to be a key member of our clinical pharmacy team.
