About the Role
We are seeking a compassionate, reliable, experienced, and dedicated Supported Living Team Leader to join our team.
The successful candidate will play a key leadership role in the day-to-day delivery of high-quality, person-centred support to adults with a range of complex needs. You will provide guidance and supervision to Support Workers while also working directly with individuals to promote independence, confidence, dignity, wellbeing, and meaningful community involvement.
You will help ensure that the service operates safely, effectively, and in accordance with individual support plans, risk assessments, safeguarding procedures, company policies, and relevant health and social care standards.
The Team Leader will act as a positive role model for the staff team, supporting colleagues to deliver consistent, respectful, and high-quality care and support.
About the Service
We provide supported living and community-based support to adults with a variety of needs, including:
- Learning disabilities
- Autism spectrum conditions
- ADHD
- Cerebral palsy
- Mental health needs
- Complex support needs
- Behaviours that may present challenges
You will work alongside individuals, their families, Support Workers, healthcare professionals, social workers, and other relevant professionals to ensure that support is safe, personalised, consistent, and focused on achieving positive outcomes.
Pay Rates
Team Leader Pay: £14.50–£20.00 per hour
Actual rates may vary depending on experience, qualifications, responsibilities, shift patterns, and service requirements.
Key ResponsibilitiesTeam Leadership and Supervision
- Provide day-to-day leadership, guidance, and support to Support Workers.
- Act as a positive role model and promote high standards of professional practice.
- Coordinate staff duties and ensure appropriate staffing levels across shifts.
- Lead shifts and ensure that daily routines and planned activities are completed safely and effectively.
- Delegate responsibilities appropriately while maintaining oversight of the service.
- Support new staff during induction and help them understand individual support needs, policies, procedures, and expected standards.
- Provide coaching, mentoring, and constructive feedback to team members.
- Identify training and development needs and raise these with management.
- Support staff to maintain professional boundaries and positive working relationships.
- Escalate staff performance, conduct, attendance, or capability concerns to management in accordance with company procedures.
- Promote effective communication and teamwork within the staff team.
Person-Centred Support
- Ensure individuals receive personalised support based on their needs, preferences, goals, and support plans.
- Promote independence, dignity, choice, privacy, and respect at all times.
- Ensure staff understand and consistently follow individual support plans and risk assessments.
- Build positive and trusting relationships with individuals.
- Encourage personal growth, confidence, and independence.
- Support individuals to make informed choices and have control over their daily lives.
- Monitor whether support is achieving positive outcomes and report any changes or concerns to management.
- Encourage meaningful involvement in the local community.
Daily Living Support
- Oversee and, where required, provide support with daily living activities including:
- Cooking and meal preparation
- Cleaning and household tasks
- Shopping and budgeting
- Developing independent living skills
- Managing daily routines
- Ensure individuals live in a safe, clean, comfortable, and well-maintained environment.
- Provide or oversee personal care support where required and in accordance with individual plans.
- Encourage individuals to develop and maintain practical life skills.
Health and Wellbeing
- Monitor the health and wellbeing of individuals and ensure concerns are appropriately identified, documented, and escalated.
- Oversee medication support and administration in line with training, policies, medication protocols, and individual care plans.
- Ensure medication records are completed accurately and appropriately.
- Report medication errors, missed medication, health concerns, and other incidents promptly.
- Liaise with healthcare professionals and other relevant services where required.
- Ensure staff follow health and safety procedures.
- Promote healthy lifestyles, emotional wellbeing, and positive relationships.
Safeguarding and Risk Management
- Promote and maintain a strong safeguarding culture within the service.
- Ensure safeguarding concerns are recognised, reported, documented, and escalated in accordance with company procedures and statutory requirements.
- Ensure staff understand and follow safeguarding procedures.
- Monitor and review risk assessments and report changes in individuals' circumstances.
- Support the implementation of appropriate risk management strategies.
- Respond appropriately to incidents, accidents, complaints, and emergencies.
- Ensure accurate and timely incident reporting.
- Escalate serious incidents and safeguarding concerns to the Registered Manager or appropriate senior management.
Supporting Positive Behaviour
- Support staff to respond calmly, consistently, and professionally to behaviours that may challenge.
- Ensure positive behaviour support strategies are followed consistently.
- Promote proactive approaches that reduce distress and support positive outcomes.
- Ensure individuals are treated with dignity and respect at all times.
- Monitor incidents and identify potential patterns or triggers.
- Ensure incidents involving behaviours that may challenge are accurately recorded and reported.
- Work with managers and relevant professionals to implement appropriate support strategies.
Community Access and Independence
- Encourage and oversee opportunities for individuals to participate in:
- Social activities
- Hobbies and interests
- Education and training
- Volunteering
- Employment and work opportunities
- Community activities
- Appointments and essential services
- Promote independence and meaningful choice.
- Support individuals to develop confidence and skills to access their local community safely.
- Ensure staff actively promote inclusion rather than creating unnecessary dependence on support.
Documentation and Record Keeping
- Ensure daily records, handover notes, incident reports, medication records, and other required documentation are completed accurately.
- Review records to ensure they are clear, factual, person-centred, and completed in a timely manner.
- Maintain confidentiality and ensure information is handled in accordance with company policies and data protection requirements.
- Ensure important information is communicated effectively during staff handovers.
- Report missing, inaccurate, or poor-quality documentation to management.
Quality and Compliance
- Support the Registered Manager and senior management team in maintaining high standards of service delivery.
- Assist with internal audits, quality checks, and service reviews.
- Monitor compliance with support plans, risk assessments, medication procedures, health and safety requirements, and company policies.
- Identify areas where service quality can be improved and communicate recommendations to management.
- Support the service in meeting relevant regulatory and contractual requirements.
- Ensure staff maintain professional standards and follow company policies and procedures.
- Contribute to a positive culture of continuous improvement.
Communication and Partnership Working
- Maintain effective communication with individuals, families, Support Workers, managers, social workers, healthcare professionals, and other relevant agencies.
- Attend or contribute to meetings relating to individuals' care and support where required.
- Communicate changes in individuals' needs, risks, health, behaviour, or circumstances promptly.
- Promote positive relationships between the service, individuals, families, and external professionals.
- Ensure information is communicated accurately and professionally.
Essential Skills and Knowledge
Applicants should have:
- Previous experience working in health and social care, supported living, residential care, or a similar setting.
- Experience supporting adults with complex needs.
- Experience working with or supervising care/support staff.
- Good understanding of safeguarding principles and procedures.
- Understanding of person-centred planning and support.
- Awareness of autism, ADHD, learning disabilities, cerebral palsy, mental health needs, and complex support needs.
- Understanding of positive behaviour support approaches.
- Strong verbal and written communication skills.
- Ability to lead and motivate a team.
- Ability to prioritise tasks and manage competing demands.
- Ability to remain calm, patient, professional, and consistent under pressure.
- Good organisational and record-keeping skills.
- Ability to work independently and make appropriate decisions while knowing when to escalate concerns.

Desirable Experience and Qualifications
- Previous experience as a Team Leader, Senior Support Worker, Senior Care Worker, or similar role.
- Relevant Level 2 or Level 3 qualification in Health and Social Care.
- Experience supervising or mentoring staff.
- Experience supporting individuals with complex needs.
- Experience managing or supporting behaviours that may challenge.
- Knowledge of positive behaviour support.
- Experience with medication administration and medication records.
- Experience with safeguarding investigations or incident management.
- Experience completing or reviewing risk assessments and support plans.
- Experience working with multidisciplinary teams.
- Leadership or management training.
