This 12-month contract role is based at Townsend Hub, a 35-bed reablement unit in central Liverpool. The Reablement Assistant plays a crucial role in supporting service users to regain and maintain their independence through person-centred care and structured reablement programmes. The position involves collaborative work with families and multi-disciplinary teams to ensure comprehensive support.

Support service users with their physical, social, and emotional needs in line with care plans.

Assist individuals with daily living tasks and promote independence.

Work with individuals to build and maintain essential life skills.

Encourage independent living through activities such as cooking, household tasks, and personal care.

Deliver care in line with individual person-centred support plans.
-
Participate in assessments and reviews as required.

Maintain accurate records and report significant changes to the Line Manager.

Administer medication where required (community-based work).

Collaborate with health and social care professionals.
-
Liaise with external services to minimize risks and support independence.

Participate in team meetings, training, and supervision sessions.

Follow safeguarding procedures for vulnerable adults.

Conduct and contribute to risk assessments where appropriate.

Ensure health and safety standards are maintained at all times.

Support individuals with recreational, social, and therapeutic activities.

Assist in service delivery within the onsite café environment where required.

Hold keys to service users’ homes where necessary, ensuring security procedures are followed.

Participate in flexible working patterns across a 7-day rota (8am–10pm).

Combination of unit-based and community support.

Physical role involving manual handling and prolonged standing.

Essential:

NVQ Level 3 in Health & Social Care (or willingness to work towards).
Desirable:

Manual Handling certification.
