We are seeking a exceptional individual to join our Dermatology Skin Cancer team as a Macmillan Cancer Care Co‑ordinator, delivering best‑practice personalised care and support for people living with and beyond skin cancer as part of our Living with and Beyond Cancer (LWBC) Programme.
In this role, you will work closely with and in collaboration with the Skin Cancer Clinical Nurse Specialists (CNS), supporting patients and their families throughout their cancer pathway. You will help patients to navigate services and manage the practical, emotional and psychosocial impact of a skin cancer diagnosis and treatment.
You will be part of the Trust Cancer Care Co-ordinator teams, helping to embed and integrate personalised care across skin cancer pathways, ensuring that support is accessible, timely and tailored to individual need. You will also work with the wider LWBC team to support the planning and delivery of Health and Wellbeing events and other personalised care interventions.
We are looking for someone who can work autonomously, confidently make decisions in a variety of situations and communicate effectively and works well as part of a multidisciplinary team.
The Macmillan Cancer Care Co-ordintor will form an integral part of the multidisciplinary team approach in caring for the physical, social and psychological needs of patients diagnosed with cancer and their carers throughout the care treatment pathway and after care support. The post holder will work in close unison with the Clinical Nurse Specialists to ensure the coordination and management of patients at all points of their treatment but in particular the Co-ordinator will also act as a point of contact for patients who will have been prepared by their clinical team for Patient triggered Follow Up (PTFU) - and will be able to sign post patients to support their self-management
The post holder will work independently within the remit of the role but under the guidance and supervision of the Clinical Nurse Specialists.
