An exciting opportunity has arisen for a Social Inclusion Worker working within Birmingham Early Intervention in Psychosis Service. The post is based at Finch Road Primary Care Centre. The successful candidate will take a lead in the development and provision of social/therapeutic activities for service users who are under the care of Birmingham Early Intervention in Psychosis Service.
You will work very closely with the Multi Disciplinary Team and have the opportunity to contribute to assessments and interventions, through community activities and social interaction. You will be expected to facilitate and participate in planned and ad hoc group work and individual activities with service users, in addition to this you will be required to motivate and negotiate with service users who may be unwilling to participate in activity and to promote the benefits and outcomes that participation could generate, i.e. social interaction, well-being and independence.
- Support the Multi-disciplinary Team in engaging service user and parent/carer’s involvement with the service.

- To provide practical support and social engagement with the service user.
- To accompany service users as required to access health and therapeutic activities under the guidance and supervision of the MDT.
- Responsible for reporting and recording any changes in the service user’s mental/physical/emotional state to the care co-ordinator or senior clinician.

Clinical
- To assist the Multi-disciplinary Team in the assessment, planning, implementation and evaluation of the evidence based care required, including health promotion for a named service user/group.
- To act as a co-worker under the supervision of the clinical team.
- To assist the Multi-disciplinary team in the setting of quality standards, including the monitoring and reviewing in line with current clinical guidance practice and policy.
- Responsible for reporting and recording any changes in the service users mental/physical/emotional state to the care co-ordinator or senior clinician.
- Working to the direction of the senior practitioners to ensure that the requirements of the care plan are observed, adhered to and implemented.
- Support the Multi-discipline team in engaging service user and parent/carer’s involvement with the service.
- To assist the care co-ordinator in the establishment of therapeutic relationships with service users, and the implementation of evidence based therapeutic interventions with appropriate boundaries.
- To take a lead role in the development and provision of social/therapeutic activities for service users.
- To provide practical support and social engagement of the service user.
- To escort service users as required to access health and therapeutic activities under the guidance and supervision of the MDT.
- Assist service users in meeting their Activities of Daily Living.
- To participate in multi-professional reviews and team meetings.
- Perform delegated clinical/non clinical duties without the need for direct supervision.
- To deliver Social Inclusion Projects.
- Build relationships with the VCS to deliver bespoke packages.
- Ensure that achievement goals are measured and driven by the service users care plan.
Professional
- Responsible for participation in the Trust appraisal process, that identifies in conjunction with appraiser your own personal development and training needs.
- To participate in research, service modernisation, clinical governance and the National Service Framework as appropriate under the guidance of the Team Manager.
- To respect and value the different therapeutic approaches within the multi-disciplinary team but ensure that the agreed philosophy approach is adopted and followed.
- Responsible for ensuring confidentiality is maintained at all times in accordance with the data protection act Trust policy and good practice and Freedom of Information act.
- Responsible for maintaining and conducting oneself in a professional manner towards service users, carers, colleagues and other agencies.
- Participate in joint working with appropriate experts/agencies.
Education
- To assist in the supervision/instruction of students on placement/induction within the clinical area under the guidance and advice of the clinical team.
- To attend clinical supervision in accordance with Trust policy.
- To attend the BSMHFT induction programme and ongoing team training.
- To attend mandatory training and keep abreast of developments.

- The post holder is responsible for ensuring that they clearly communicate with service users and carers, actively listen to establish an understanding of the needs of the service user and their carers, enabling them to make informed decisions about their care.
- For maintaining efficient recording of visits to service users on RiO for audit and statistical purposes.
- Responsible for maintaining appropriate communication with other teams, to service users, inpatient units.
- Responsible for handing over work completed in handover meeting.
- Responsible for effective high standards of patient care under the direction of the Multi-Disciplinary Team within the scope of the post.
Managerial
- To work within a framework commensurate with their level of knowledge and competence and to act accordingly within their sphere of responsibility.
- To manage own activity and workload delegated, ensuring work carried out is handed over to the team.
