Your new Nurse Practitioner role:
We are offering an exciting opportunity for a Nurse Practitioner to join us 4 days per week. You will be supported by our friendly team of clinicians; 6 GP partners, 6 salaried GPs and a large nursing team. Our clinical team work closely to support each other, and regular team meetings are supported by the practice where our clinicians can communicate and suggest new and improved ways of working.
Job Purpose:
The Nurse Practitioner (NP) will act autonomously within their professional scope of practice to provide care for patients through face-to-face appointments, telephone triage and home visits.
Operating within their own sphere of competence and agreed acceptable limits of practice the NP will provide expert professional clinical advice to patients, carers and colleagues, ensuring clinical safety and excellence.
The NP will collaborate with a multidisciplinary team working with secondary care, admissions avoidance, and within minor illness.
Experience in diabetes management is desirable.
Working pattern:
4 days per week (working days TBC).
Up to 30 hours per week.
