Job Overview
We are seeking a dedicated and compassionate Care Coordinator to join our team. The successful candidate will play a vital role in organising and managing appointments, allocating SystmConnects to the appropriate clinician and ensuring patients receive the highest standard of care. This position offers an excellent opportunity for individuals with strong organisational skills and a passion for helping others to make a meaningful difference in people's lives. Prior general practice experience is essential, although full training will be provided.
Duties
- Coordinating patient appointments and managing daily clinical schedules
- Processing and triaging SystmConnect requests to the appropriate clinician
- Supporting patients with enquiries, providing guidance and reassurance
- Liaising with GPs, nurses, and external healthcare providers
- Updating and maintaining accurate patient records and clinical data
- Coding patient information in line with QOF requirements
- Managing correspondence, results, and pathology requests via ICE
- Supporting the wider team with administrative duties and daily operations
- Ensuring confidentiality and data security at all times
- Assisting with opening/closing the practice and maintaining security procedures
- Provide reception cover as and when needed
Requirements
- Previous general practice experience is essential, although full training will be provided.
- Excellent communication skills, both written and verbal.
- Strong organisational abilities with attention to detail.
- Compassionate approach with a genuine interest in supporting others.
- Ability to work independently as well as part of a team.
- Proficiency in basic computer applications such as Microsoft Office (Word, Excel, Outlook).
- Flexibility to adapt to changing priorities and patient needs. This role is ideal for a flexible, motivated individual eager to contribute positively within a caring environment, supporting patients through organised and compassionate service delivery.
Working Pattern
- 30 hours per week
- Monday to Friday
- Shifts Patterns are: 08:00 – 14:00 and 12:30 – 18:30
