Outcome Care and Support is looking for a proactive and experienced part time Care Co-ordinator to join our growing domiciliary care service.
We provide person-centred care across Shropshire and the surrounding villages, supporting people to remain safe, independent and comfortable in their own homes. We’re proud of the relationships we build with our service users, families and staff, and we’re looking for someone who shares our values.
This is a varied, rewarding and hands-on role. You’ll work closely with our existing Co-ordinator and management team to help keep the service running smoothly, support our carers and make sure the people we support receive safe, consistent and high-quality care.
What you’ll be doing
Your responsibilities will include:
- Working closely with our Co-ordinator and Registered Manager, to manage the day-to-day running of the service
- Creating, reviewing and updating person-centred care plans
- Coordinating care visits and ensuring people receive the right support at the right time
- Allocating staff effectively, taking account of skills, experience and continuity of care
- Supporting with rota changes, staff availability and last-minute issues
- Maintaining clear communication with service users, families, carers and professionals
- Supporting and supervising care staff through mentoring, guidance and performance monitoring
- Completing spot checks, competency checks and quality monitoring visits
- Visiting service users to review care and respond to changes in needs
- Carrying out care calls when required to support the team and maintain continuity of care
- Monitoring care records, call times and staff performance
- Supporting assessments, reviews and risk assessments
- Ensuring records are accurate, clear and up to date
- Promoting safeguarding, dignity, choice and independence
- Helping us identify improvements and raise standards across the service
About you
We’re looking for someone who is approachable, dependable and able to think on their feet.
You’ll need:
- Experience working in adult social care or domiciliary care
- Previous senior, supervisory or care co-ordination experience
- A good understanding of person-centred care planning
- Strong communication and organisational skills
- Confidence managing rotas, priorities and changing demands
- A willingness to work both in the office and out in the community
- A practical, hands-on approach and willingness to complete care calls when needed
- Knowledge of safeguarding, health and safety and regulatory requirements
- Good IT skills and experience using electronic care planning or rostering systems
- A full UK driving licence and access to a vehicle
- A positive attitude and genuine commitment to supporting both staff and service users
Experience with the Access Care Planning system would be helpful, but training can be provided.
