We are seeking a compassionate, organised, and proactive Family Health and Safeguarding Coordinator to join our team.
As a Family Health and Safeguarding Coordinator you will be responsible for maintaining the practice safeguarding registers, identifying and escalating safeguarding concerns in line with local and national policies, and supporting the effective management of safeguarding cases.

Job Summary:
You will act as a central point of contact for our patients, carers, families and external agencies in regard to safeguarding concerns to ensure the appropriate care and support is provided for those at risk to ensure their health, wellbeing and safeguarding needs are met by working closely with our healthcare professionals and external agencies.
The role helps to support the identification and management of safeguarding concerns, arranging referrals and monitoring outcomes to provide the most appropriate care.
You will also facilitate in-house safeguarding meetings, monitor safeguarding actions and outcomes, and work closely with the multidisciplinary team and external agencies to ensure appropriate communication, information sharing, and continuity of care.
The role supports the practice in meeting its safeguarding responsibilities by maintaining accurate records, monitoring safeguarding processes, and promoting best practice across the organisation.
This role is ideal for someone who has experience of working in safeguarding, family support, health or social care and who is passionate about improving the support and wellbeing outcomes for children and families through collaborative person-centred care.
