Job Summary
We are seeking a compassionate, skilled carer support worker to work wake nights as part of the existing care team. Our client is a 19 year old female with complex needs and cerebral palsy. You will work closely with the team, case manager and wider multi disciplinary team to ensure person centred care is delivered whilst following the best practice legislation.
Experience in PEG feeding and tracheostomy & stoma care is essential.
Duties
- Assist clients with daily activities including personal hygiene, dressing, and mobility support
- Administer medication in accordance with prescribed care plans and medical guidelines
- Support clients with PEG feed preparation and nutritional needs, ensuring dietary requirements are met
- Maintain accurate documentation of care provided and any changes in client condition
- Support client with medical appointments and accompany them when necessary, including driving to appointments
- Assist with light housekeeping tasks to maintain a clean and safe environment
- Collaborate with healthcare professionals to develop and review personalised care plans
- Respond promptly to emergencies or urgent needs following first aid protocols
Skills
- Proven experience in complex care essential.
- Knowledge of medical terminology, medication administration, and case management principles
- Excellent communication skills with the ability to engage effectively with family and the MDT
- Ability to safe manual handling techniques, when required and assist with mobility support
- Valid driving licence for transporting client safely to appointments or activities essential.
- First aid certification is preferred but not essential; training can be provided for suitable candidates
- Compassionate nature with patience and empathy for vulnerable individuals requiring specialised support
