We are seeking a dynamic and compassionate Care Team Leader to join our dedicated supported living team. Duties
- Lead, motivate, and supervise care staff to provide outstanding personalized care to residents
- Develop, implement, and regularly review individual care plans in collaboration with residents and their families
- Ensure all care activities adhere to regulatory standards and internal policies
- Foster open communication within the team and with residents’ families to promote transparency and trust
- Oversee daily operations, including medication administration, activity planning, and health monitoring
- Conduct staff training sessions on best practices, health & safety procedures, and IT systems used for documentation
- Maintain accurate records of care activities, incident reports, and staff schedules
Requirements
- Proven supervising or leadership experience within a care home or senior care setting
- Strong communication skills to effectively liaise with residents, families, and team members
- Ability to develop comprehensive care plans tailored to individual needs
- Valid driving license preferred for transportation duties or home visits when necessary
- Familiarity with IT systems used for documentation and reporting in healthcare environments
- Knowledge of safeguarding procedures and regulatory compliance in adult social care
- Passionate about delivering compassionate, person-centered care that enhances residents’ quality of life.
