We are seeking a compassionate and dedicated Health Care Assistant to join our team. Responsibilities
- Assist patients with daily activities such as bathing, dressing, and mobility
- Support with medication administration as per care plans
- Monitor and record patients’ health conditions and report any concerns to senior staff
- Provide emotional support and companionship to patients
- Maintain accurate documentation of care provided using IT systems
- Follow health and safety procedures to ensure a safe environment for both patients and staff
- Drive or accompany patients to appointments or outings when required
- Assist in the development and review of individualised care plans in collaboration with healthcare teams
- Communicate effectively with patients, families, and healthcare professionals to ensure high standards of care are maintained
Skills
- Good communication skills in English, both verbal and written
- Ability to work independently and as part of a team within a care home or home care setting
- Basic IT skills for recording patient information accurately
- Strong organisational skills to manage multiple tasks efficiently
- Compassionate nature with a genuine desire to support others’ wellbeing
- Driving licence is desirable for roles involving patient transportation or home visits
- Knowledge of care plans and the ability to adapt support accordingly
