Support clients to explore and access appropriate progression opportunities, including voluntary roles and paid employment.
Complete and review wellbeing assessments and other agreed outcome measures throughout the client journey to monitor progress and demonstrate distance travelled.
Regularly review client progress against agreed goals, adapting support and action plans where required to reflect changing needs, achievements and aspirations.
Plan, prioritise and organise caseload activity effectively to meet contractual targets, service KPIs and quality standards.
Maintain accurate and up-to-date client records, including vocational profiles, action plans, progress updates, risk assessments and other required documentation.
Use caseload data and client outcomes to evidence impact and contribute to service performance reporting.
Client Support & Development
Plan and deliver tailored one-to-one activities and short interventions that develop confidence, independence, life skills and readiness for future progression.
Provide practical support in areas such as travel training, Training in Systematic Instruction (TSI), money management, budgeting, shopping, digital skills and accessing local services.

Identify and facilitate activities and opportunities that support clients to develop their skills, independence and community participation.
Support clients to access and engage with local groups, courses, activities, community venues and other appropriate services and opportunities.
Provide practical support to help clients overcome barriers to participation, encouraging greater confidence, choice and independence over time.

Other Responsibilities
Develop and maintain positive working relationships with community and statutory partners, families, carers, professionals, employers and other stakeholders to support effective service delivery and positive outcomes for clients.
Build strong links with local community organisations and services to increase opportunities for clients, support effective signposting and promote greater community participation.
Manage your own diary, workload and administration effectively, balancing caseload responsibilities, community-based delivery, travel, meetings and other service requirements.
Maintain high-quality documentation and administration, ensuring client records, case notes, correspondence, reports and data are accurate, timely and compliant with contractual, organisational and audit requirements.
