Role Purpose
To provide efficient day-to-day administration and coordination of the Hospice at Home service, ensuring the smooth delivery of patient care packages and effective support to clinical operations. The post holder will act as a key point of contact for staff, patients' families, multidisciplinary professionals, and Continuing Healthcare colleagues, facilitating communication and the timely arrangement of care services. Working both independently and as part of a team, the role includes providing telephone and office cover, managing service capacity issues, and, in the absence of the Clinical Team Leader and Staff Nurse, supporting decision-making regarding adjustments to existing care packages to maintain safe and responsive service delivery.
Key Responsibilities
Coordinate the day-to-day administration of the Hospice at Home service, including communicating and confirming care package arrangements with patients, carers, and healthcare professionals.
Respond to and resolve administrative queries relating to staff rotas, timesheets, mileage claims, mobile devices, annual leave requests, and other workforce administration matters in collaboration with the wider administration team.
Act as a first point of contact for telephone enquiries, taking messages, responding appropriately, and liaising closely with the Clinical Team Leaders and clinical staff to ensure timely communication and follow-up.
Assist with enquiries relating to the Hospice at Home service, providing accurate, timely, and professional information to patients, families, healthcare professionals, and other stakeholders.
Provide comprehensive administrative and clerical support to the Hospice at Home service, including preparing correspondence, photocopying, filing, document production, and circulation of information.
Accurately input and maintain patient information within EMIS and other relevant systems, ensuring confidentiality, data protection, and record-keeping standards are upheld at all times.
Collate, check, and process staff timesheets and mileage claims, ensuring accuracy and compliance with payroll deadlines.
Organise and attend clinical and operational meetings, preparing agendas, taking minutes, and distributing documentation as required.
