As a Band 4 Nursing Associate or Associate Practitioner you will work as part of a local team, working closely with the local acute hospital, social care, mental health teams, GPs, and Practice Nurse. You will utilise clinical skills and effective risk assessments, adhering to evidence-based policies and empowering individuals.

We are seeking a highly motivated professional t o work within a wider team of nurses and health care assistants. You will work under the direction of a District Nurse and Registered Nurses, to assess, implement and evaluate person centred care for a caseload of patients, undertaking venepuncture, complex wound care and catheterisation.
Post holders will be available to work Monday to Sunday between 8am and 8pm. A full UK driving license and vehicle for business use if required for this post, however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

- To undertake unsupervised and delegated clinical skills including administration of subcutaneous or intramuscular injections (insulin, Vitamin B12’s, Heparin), female/male/suprapubic catheterisation, glucose monitoring, wound care including compression bandaging, where appropriate, following training and completion of competencies.
- To deliver a high standard of nursing care, contributing to the ongoing assessment, providing and monitoring evidence based care under delegation from a Registered nurse, working in partnership with the multi-disciplinary team.
- Provide compassionate, safe and effective care and support to patients in the community.
- To administer medication from a limited formulary under direction of local protocol/policies. Awareness of legal, ethical and risk assessment
- To report any changes in condition of the patient to the District Nurse / Team Leader or in their absence the designated nurse in charge of the caseload .
- To have the ability to work unsupervised, prioritising and organising tasks and home visits as delegated by the nurse in charge. Ability to safely delegate tasks to others.
- To respect confidential information obtained in the course of work and refrain from disclosing such information to anyone other than professional staff directly concerned with the patients care or other authorised person.
- Support patients to improve and maintain their mental, physical, behavioural health and well-being.
- Care for a patient’s nursing needs whilst promoting comfort and well-being, ensuring their personal and social needs are met.
- Safely undertake the procedures outlined in the NMC Nursing Associate Skills Annex to meet the patient’s nursing needs to include; undertaking routine procedures including venepuncture, measuring and interpreting blood glucose levels, recording and interpreting vital signs, continence care, bladder and bowel care, pressure area care, wound care, nutrition, hydration, foot care, skin integrity, mood identification
- Assist in the assessment of and contribute to the management of risk across several areas within the environment where care is being administered
- Work with the Registered Nurse and engage with agencies and community teams to ensure a safe, individualised approach to the admission and discharge of patients
- Maintain accurate, contemporaneous patient records in line with Trust policy and the NMC code , record on EMIS and on clinical paper records
- Provide effective evidence based care for service users under the indirect supervision of Registered Nurses in order to perform non-complex activities, in line with clinical policies and professional standards.
