The social work team is based within a multi-disciplinary team co-located with various health colleagues working together to provide and commission effective support to adults with learning disabilities.
We have a full-time opportunity (37hrs per week) for a Practitioner Manager (Transition) in the Learning Disability (LD) team.

About the role
Pay Details: Grade 10 SCP 36 – 39 £47,665 - £51,928
Hours of Work / Working Pattern: 37 hours per week
Main Place of Work: Hen Goleg, College Fields Close, Barry, CF62 8LF
Description: You work in the Learning Disability team based at Hen Goleg in Barry. We operate hybrid working allowing you to work from home and in the office. Concrete arrangements will be agreed between you and your supervisor.
You will be responsible for the professional oversight of a team of Social Workers, Support Planners, and Reviewing/Social Care Officers who are responsible for the commissioning of care and support to people with learning disabilities and their families.
About you
You will need:
. Appropriate Professional Social Work Qualification
- Current Registration with relevant Professional Body
- Extensive statutory Social Work experience, working in a multidisciplinary Assessment
and Care Planning environment.
- Experience of working with people with a learning disability including those with a
profound and multiple learning disability
- Experience of working in partnership with other Agencies
- Supervisory experience (e.g. mentoring, practice teaching)
- Experience of managing Adult/Child Safeguarding cases and Knowledge of legislation
and guidance.
- Experience of service quality monitoring.
- Experience of applying a strength-based approach in your daily work.
