We are looking for a motivated, proactive, and enthusiastic Specialist Pharmacist to lead our pharmacy team in developing and delivering of a high-quality, specialised pharmacy service for the Inflammatory bowel Disease (IBD) and Homecare.
This is an exciting opportunity to join our dynamic, innovative, and friendly team dedicated to improving IBD services. In this key role, you will work as part of an established multidisciplinary team and be responsible for leading and shaping high-quality clinical pharmacy services in IBD and homecare services. You will play a crucial role in patient education, monitoring, and safe prescribing practices, ensuring adherence to national guidelines and local policies
Lead the development and delivery of a specialist Inflammatory Bowel Disease (IBD) pharmacy service across the Trust, working closely with primary and secondary care providers to optimise patient outcomes.
Provide expert advice on the prescribing, monitoring and optimisation of biologics, biosimilars, small molecules, immunomodulators and other therapies used in IBD.
Lead pharmacist-led outpatient clinics to initiate treatment, assess treatment response, safety, adherence and therapeutic optimisation.
Oversee and coordinate pharmaceutical care for patients with IBD across inpatient, outpatient and day-unit settings.
