About the Role
As a Dementia Practitioner, you will play a vital role in delivering specialist dementia care and support to our residents. Working closely with the Registered Manager, you will be responsible for:

Providing expert advice and guidance to colleagues and carers on dementia care
- Conducting assessments and reviews of individuals' needs
- Implementing person-centred care plans to ensure the highest standards of care are met
- Supporting individuals and their families through the challenges of living with dementia
- Promoting awareness and understanding of dementia within the organisation and the wider community
Please note your working pattern would be Saturday, Sunday and Monday every week. 12 hours per shift, 7am-7pm.
