The role:
The Advanced Diabetes Specialist Nurse is a registered nurse with significant experience, skills and post registration education in the field of diabetes.
The post holder will provide clinical leadership operationally and clinically manage a defined Diabetes Team reporting to the Service Manager.
Whilst there is no direct financial responsibility with this post, the post holder will ensure effective utilisation of resources within the service including equipment, supplies and prescriptions.
Receive and act upon clinical referrals from both primary and secondary care.
Manage a complex specialist caseload of patients with diabetes, making highly complex clinical decisions. This includes comprehensive assessments, development of individualised person centred care plans and ongoing treatment delivery for patients referred to the service across a range of community care settings.
Delegation of a defined caseload of patients within the diabetes team in line with patient need and scope of practice whilst having overall responsibility for the delivery of specialist diabetes care.
Work with multidisciplinary teams to develop, deliver and evaluate structured evidence based diabetes education programmes for patients and carers.
Provide specialist educational expertise to healthcare professionals and other voluntary groups/agencies acting as a professional resource to these groups.
Contribute to the development of local, Board wide and National guidelines/standards appropriate to diabetes. Partake in reviewing and updating these guidelines which influence clinical practice locally board and national.
Collaborate with other disciplines/ professions in the development and participation of research and audit to ensure effective delivery of service.
The job requires driving as part of day to day work, PVG scheme will be required to be obtained
You need to have a live NMC UK pin in order to be considered for this post.

Key responsibilities:
- Assess, plan and develop individualised care plans for patients who have diabetes and their families/carers in particular for those who require complex diabetes management.

Undertake high level clinical interventions at various stages of life for individuals living with diabetes; including Insulin initiation, insulin dose adjustment, GLP-1 initiation and supporting development of the skills necessary for self-management.

Provide Tele-Health consultations and advice in response to patient enquiry and need, enabling direct access to specialist advice to enhance self-management and minimise risk for patients at times of crisis or during periods of illness when risk of hospital admission/re- admission is greatest.

Deliver patient care based on principles of best practice and patient self-care, challenging and improving current practice as appropriate; ensuring practice is evidenced and research based.

Support patients through difficult life experiences that may affect their diabetes control.

Enable patients to set realistic targets for glycaemic control using a person centred individualised approach optimising glycaemic control with life-style advice that meets their specific needs.

Ensure that all written documentation within the department area is clear, concise and timely and complies with professional and organisational standards

Provide highly specialist expertise for patients and wider nursing services to develop personalised diabetes management plans for insulin administration and ketone management and specific treatment guidelines for end of life diabetes care.

Knowledge, training, qualifications and/or experience required to do the job:
- Registered Nurse (Adult) on NMC Register with significant experience in Diabetes care with a portfolio of evidence demonstrating achievement of level 7 core Advanced CNS competencies and specialist diabetes competencies necessary for the role. This should include:
- An appropriate post graduate diploma (masters level) (or 120 credits equivalent)
- workplace based assessments, practice related feedback, and reflective accounts

Evidence of Continuing Professional Development.

Competent in the use of technology e.g. IT, PCs and software.

Highly effective communication skills, verbal and written.

Highly developed problem solving skills.

Non-medical prescriber (V300).

Ability to respond to the demands of the post with flexible working as required.

Ability to lead and manage a team.

Willing to undertake further training.
