This is an exciting opportunity for a band 3 support worker post working in Dementia and Older People's Mental Health Liaison covering Maternity leave. It would provide an experience of delivery of care to people with Dementia and mental health diagnosis.
You will be involved in the assisting the team in the delivery of care to people in both our Community hospitals and Acute Hospitals around Cornwall, assisting physical health wards in delivering the quality of care Mental Health support that is required for this most vulnerable group of patients.
You will work closely with other team members and the people delivering the care on the wards.

Seeing patients on the ward offering emotional support and advice, helping wards to meet needs, encouraging food and fluid intake, activities and getting to know the patient so that there hospital journey is optimised meaning better outcomes for the patient.
