We are seeking a compassionate and motivated Community Assistant Practitioner to support patients in their own homes, care homes and community settings. Working as part of an integrated multidisciplinary team, you will deliver rehabilitation, long-term condition and palliative care support, helping people maximise their independence and avoid unnecessary hospital admissions.
Applicants should hold an NVQ Level 3 and Foundation Degree (or equivalent experience), have experience in health or social care, excellent communication skills, and a full UK driving licence with access to a vehicle. To contribute in the delivery of high standards of health care to patients within their own homes, care homes, the travelling community, Community Hospitals, and clinics, by assessing, planning, implementing and evaluating packages of care.
