Community Registered Nurse – Nursing+ Service

Fuse Health is looking for a compassionate, organised and confident Registered Nurse to join our Community Nursing Service in Derby.
This is a part-time community nursing role supporting patients to receive safe, responsive and personalised care outside hospital.
You will work closely with patients, families, GPs and other healthcare professionals. You will work with a high level of professional responsibility and help develop a growing community service.
We are recruiting for between 15 and 22.5 hours per week and welcome a conversation about the working pattern that would suit both the successful candidate and the service.
About the service:
This community nursing service is designed to provide accessible, high-quality nursing care closer to patients’ homes.
The service works alongside primary care and the wider multidisciplinary team to help patients receive coordinated, proactive and compassionate care in the community.
The role
As a Community Registered Nurse, you will:
- Assess patients’ nursing needs in community settings
- Develop, deliver and review individual care plans and perform long term condition reviews
- Provide safe, evidence-based nursing interventions
- Monitor patients’ conditions and respond appropriately to changes
- Support patients and families to understand and manage their health
- Identify deterioration, escalate concerns and arrange appropriate clinical review
- Work collaboratively with GPs, community services and other healthcare professionals
- Maintain clear, accurate and timely clinical records
- Support continuity and coordination of care
- Promote independence, prevention and self-management
- Contribute to clinical governance, audit and service improvement
- Work within your professional scope of practice and the NMC Code

Essential requirements
You must have:
- Current registration as a Registered Nurse with the Nursing and Midwifery Council
- Relevant post-registration nursing experience
- Evidence of continuing professional development
- Sound clinical-assessment and decision-making skills
- The ability to recognise deterioration and escalate concerns appropriately
- Strong communication and record-keeping skills
- The ability to manage your workload safely
- The ability to work both independently and as part of a multidisciplinary team
- The ability to travel between community locations in Derby
- A commitment to safeguarding, infection prevention and patient safety
Desirable experience
It would be helpful to have experience in one or more of the following:
- Community nursing
- Primary care
- Urgent care
- Long-term-condition management
- Care of older people
- Wound care
- Medicines administration
- Health promotion and prevention
- Palliative or end-of-life care
- Supporting patients with complex needs
- Working across organisational boundaries
- Audit, quality improvement or service development

Working arrangements
- Hours: Between 15 and 22.5 hours per week
- Location: Community settings across Derby
- Clinical base: Derby
- Contract: Fixed Term with a view to extension at end of term
We are open to discussing how the contracted hours could be arranged while ensuring that the service and its patients are appropriately supported.
Salary
The full-time-equivalent salary is £35,000–£39,000 per year, depending on experience and any other agreed appointment criteria.
The final contracted hours and salary will be confirmed in the employment offer.
