- Administer medication following prescribed instructions and document all care activities accurately
- Communicate effectively with patients, families, and healthcare team members to ensure coordinated care
- Use IT systems to update records, track patient progress, and manage schedules efficiently
- Drive between different locations if required for home care visits, ensuring timely and reliable service
- Maintain a clean, safe environment by adhering to health and safety protocols at all times
Experience
- Previous experience working in a care home or providing home care is highly desirable but not essential; training will be provided for the right candidate
- Strong communication skills in English to interact confidently with patients and team members
- Basic IT literacy to manage electronic care records and documentation effectively
- Valid driving license preferred for roles involving travel between locations
- Ability to follow detailed care plans accurately and adapt to changing needs with flexibility
