An exciting opportunity has become available for a Band 6 Practitioner to join our award-winning team in CCETT South, working within a busy crisis and home treatment service covering the south of the county.
The team are well established and provide crisis, home treatment and enhanced treatment to children and young people experiencing a deterioration in their mental health, and in some cases at risk of hospital admission.
You will need to be able to drive as work is across the county, and based on the team sharing a caseload of CYP. CYP urgent care also works closely with our CAMHS MHLS who are based between CCETT and the acute hospitals and has a range of Multi-disciplinary professionals working across Urgent care to support our most vulnerable young people who are at risk of admission.
Shift co-ordinator role within the team- based on allocation of role fairly among senior practitioners.
Assessment, liaison, risk and care planning skills.
Ability to work with CYP and their families to ensure a collaborative approach to care. Home treatment and provide therapy and risk management in the community.
Access and provide supervision to peers.
Attend MDTS, business and team meetings and engage proactively in discussion about service development.
Provide daily leadership to peers and support the team with managing high risk cases.
