Responsibilities
- Deliver personalized care tailored to each client’s individual care plans, including assistance with daily activities such as bathing, dressing, and mobility.
- Support clients with complex health conditions by monitoring symptoms and administering prescribed medications accurately.
- Maintain clear and detailed documentation of care provided, updates to care plans, and client progress using IT systems.
- Communicate effectively with clients, family members, healthcare professionals, and team members to ensure coordinated and consistent care delivery.
- Drive between different client locations in accordance with schedules, ensuring timely support while adhering to safety protocols.
- Assist clients with home management tasks such as meal preparation, light housekeeping, and maintaining a safe living environment.
- Participate in ongoing training to stay updated on best practices in home care and complex health management.
Qualifications
- Previous experience working in a care home or providing home care services is highly desirable.
- Strong communication skills in English to effectively interact with clients and team members.
- Valid driving license and ability to travel between client locations confidently and safely.
- Familiarity with creating and following detailed care plans tailored to individual needs.
- Basic IT skills for documentation, updating records, and communicating via digital platforms.
- Ability to demonstrate excellent interpersonal skills, patience, and empathy when supporting vulnerable individuals.
- Knowledge of health and safety regulations relevant to home care environments is advantageous.
