An exciting opportunity has arisen for bank Youth Transition Officers to support our staff team.
You will be responsible for providing day to day care for our Young People, whether this is supporting them in activities, education or statutory meetings relating to their time in residential care. Creating an environment and atmosphere where children can feel safe and lead a happy life.
Purpose of Post: To support colleagues in creating a nurturing environment that affords young people the opportunity to develop intellectually, emotionally, socially, physically and spiritually.
The Main Duties Are:
· To provide young people with opportunities to develop a sense of independence, autonomy and responsibility within a structured plan.
· To assist the young people to develop a sense of identity and positive images of themselves.
· To provide young people with emotional support and assistance in a safe supportive, homely and stimulating environment.
· To positively encourage engagement in education employment and training and make effective use of available resources.
· To develop the skills, knowledge and expertise necessary in order to respond to the individual and varying needs of the young people in the home.
· To encourage young people to build and maintain positive relationships and links in the community.
· To maintain a good communicative and co-operative relationships with parents, field social workers, young persons advisers, consultant staff and others concerned with the welfare of the young people.
· To keep detailed records on the day-to-day activities of the young people.
· To contribute in the review, assessment, planning meetings and case conferences etc, for young people, and accordingly ensure that relevant reports are written and made available to other agencies concerned.
· To actively promote and facilitate independent life skills training for young people including health life styles budgeting and practical aspect of daily living.
· To contribute to the development on individual Transition (placement support and pathway plans) plan to assist young people in the process of leaving care.
· To participate in the key worker approach and share responsibility for it’s effectiveness.
· To accept responsibility, whenever applicable, for monies needed to carry out duties.
· Work in a flexible manner so that when the occasion arises that tasks not included in the job description have to be undertaken. Job descriptions will be reviewed periodically.
· To attend, participate in team meetings, supervision, training and conferences on behalf of Youth Reality Homes.
