Job summary
Our teams across Portsmouth are looking for a Community Healthcare Support Worker to work within the multi-disciplinary Integrated Care Team that delivers community based services to a GP Practice population, undertaking nursing tasks within agreed parameters, set out by the registered practitioners.
The role will include home visiting and lone working without direct supervision.
The scope of the role will focus on supporting admission avoidance, early supported discharge and promotion of independence and assisting in the effective management of patients with long term conditions and older people with mental health problems.
Full UK driving licence and access to a car is essential.
At present, the Trust is unable to offer Skilled Worker visa sponsorship for this post. Applicants must have the right to work in the UK at the time of application.

Main duties, tasks & skills required
To provide a supporting role to registered practitioners in the Integrated Community Team in the delivery of nursing care.
To observe, monitor and record patient activity and health status, communicating any changes in their condition to the registered practitioners.
To be responsible for progressing patient's treatment plans within guidelines set by registered practitioners and contributing to the review of each patient's progress within the team.
To perform a range of clinical nursing skills as directed by the registered practitioners:
Skin care/Pressure Ulcer prevention
Vital Signs - Temp/Pulse/Lying and Standing BP/Respiratory Rate
Mouth care/Eye care/Toe nails and Finger nails
Catheter care/Bladder washouts/Stoma care
Catheterisation (under registered practitioner discretion - NVQ III Training)
Bowel care (suppositories)
Venepuncture
Basic Wound care
Simple bandaging and paste bandaging
To monitor the well-being of vulnerable older people within current case load, contributing to protecting those individuals whose health and well-being is at risk.
To monitor and maintain health, safety and security of self and others in own work area, identifying and assessing potential risks and taking appropriate action, and adhering to Trust and team's health and safety policies and procedures. This job may include exposure to unpleasant working conditions or dangerous environment: unhygienic environment, bodily fluids, unpleasant smells.

Job description
To provide a supporting role to registered practitioners in the Integrated Community Team in the delivery of nursing care respecting patients' privacy and dignity at all times.
To act as key worker for patients under the direction of the registered practitioners.
To assist in planning, delivering and evaluating programmes of care prescribed by the registered practitioners, regularly liaising at the beginning and throughout each shift.
To observe, monitor and record patient activity and health status as appropriate, communicating any changes in their condition to the registered practitioners.
To be responsible for progressing patient's treatment plans within guidelines set by registered practitioners and regularly contributing to the review of each patient's progress within the team.
To demonstrate the ability to communicate complex and sensitive information in an understandable form to chronically sick and seriously ill patients, patients with enduring mental illness, carers and other staff, including imparting unwelcome news.
To ensure timely and effective communication with appropriate team members and staff in other health, social care and voluntary agencies, on both clinical and professional matters.
To act in accordance with the Trust's Record Keeping Policy, maintaining contemporaneous paper and electronic patient records. This entails the good standards of communication both in written and oral form, within the multidisciplinary team.
To assist in patient's clerical administration under the direction of the qualified clinicians and to accurately record all clinical findings and observations at all stages of the treatment programme.
To monitor equipment for the safety of self and others. Report to designated staff member when repairs or servicing are required.
To report clinical incidents and accidents to immediate superior as soon as possible and to be competent in the use of the Trust's clinical incident system to record the incident.
To be aware of the legal requirement to report any crime observed through the course of the job, this may include becoming aware of stolen goods.
To develop own knowledge and skills, achieving the competencies assigned to the role by actively participating in learning and contributing to the development of others.
To attend all relevant mandatory and essential training.
Attend all relevant inter-disciplinary meetings as directed.
To participate in the induction programme of all new staff.

Current (UK/EU) Driving Licence is required, with access to own vehicle for work purposes.
