Duties & Responsibilities
Nottingham City’s Young Persons accommodation service (NCYPAS) is recruiting for a support planner. We have 31 self-contained flats across two properties with one dispersed property out in the community. We work with 18–21-year-olds who have been assessed as homeless but have the capacity to work with 16/17-year-olds and care experienced people up till the age of 25. With the service manager, assistant manager, other planners and workers you’ll be working with young people in finding permanent accommodation and supporting with any barriers they encounter. You'll look to create support plans and risk assessments, along with independence plans to find barriers and seek solutions.
We're looking for a strengths-based approach, using the elements of being psychologically informed and creating environments where our young people will flourish. We want creative and innovative solutions to work with our young people. Our key partner is Housing Solutions and that’s where all our young people are referred from. We are given a 6-month time frame to work with them. The role will be fast paced but will be an exciting opportunity!
37 hours a week working every *other weekend, both with morning and afternoon shifts. * We have recruited weekend workers, so this will change to one weekend in every 3 weeks.

Assist service users with housing applications and move on options, using the new support plans and risk assessments.
- Work with the service manager, assistant manager, planners and workers in supporting our young people.
- Appreciate the value in psychologically informed environments.
- Support plan and risk assess using independence plans with a strengths-based approach.
- Sign post young people onto appropriate services for substance use management & health.
- Assist young people with basic budgeting and life skills to support them to move on and to live independently.
- To liaise appropriately with key partners, stakeholders and service providers to ensure appropriate support is offered.
- To be responsible for assessing and evaluating the needs of service users, recording relevant information specific to a person’s support needs and co-ordinating referrals into the service.
