Are you passionate about supporting people to live independently and achieve the outcomes that matter most to them? Do you enjoy working creatively, building strong relationships, and making a real difference in your local community?
We are seeking enthusiastic, compassionate and motivated UK-based Assistant Practitioners to join our dynamic Older People and Physical Disability (OP/PD) Team within Adult Social Care.
This is an exciting opportunity to work alongside experienced Social Workers, Occupational Therapists and Assistant Practitioners within a forward-thinking service that is embracing innovative and strengths-based ways of working.
About the Role
As an Assistant Practitioner, you will play a key role in supporting adults with physical disabilities and older people to maintain their independence, improve their wellbeing, and achieve their personal outcomes.
You will undertake assessments, reviews and support planning, identify preventative and community-based solutions, and work closely with individuals, carers, families and partner agencies to ensure people receive the right support at the right time.
The role provides an excellent opportunity to build professional knowledge and experience whilst making a meaningful difference to people’s lives.
Our service is actively developing approaches such as:
- Community Social Work
- Human Learning Systems
- Community Led Support (CLS)
These approaches focus on early intervention, prevention, collaboration and creativity, helping people to draw upon their personal strengths, support networks and community resources before reliance on formal services becomes necessary.
About the Team
You will be joining a highly experienced, welcoming and supportive Older People and Physical Disability Team, committed to delivering outstanding person-centred support.
Our team supports adults who may be experiencing a wide range of complex circumstances, including physical disabilities, long-term health conditions, frailty, social isolation, self-neglect, safeguarding concerns and changing care and support needs.
We are proud of our positive team culture, where learning, development and collaboration are actively encouraged. Practitioners support one another through regular supervision, peer learning and shared problem-solving, ensuring that no one feels alone when managing complex situations.
What We Are Looking For
We are looking for individuals who are:
- Passionate about supporting people to remain as independent as possible.
- Skilled communicators who can build effective and trusting relationships.
- Creative, curious and solution-focused in their approach.
- Organised and able to manage competing priorities effectively.
- Confident using IT systems and electronic recording systems.
Committed to delivering high-quality, person-centred practice.

Why Join Us?
In our OP/PD Team, we believe in seeing the person, not the problem. We recognise the strengths, skills and potential that every individual brings and work alongside them to achieve positive outcomes.
You will be joining a service that values innovation, continuous learning and professional development, where your ideas are welcomed and your contribution genuinely matters.
If you are enthusiastic about supporting older people and adults with physical disabilities, enjoy working collaboratively, and want to be part of a team that is passionate about making a difference, we would love to hear from you.
