About the Role
Willstac Home Care is looking for compassionate, reliable and committed Support Practitioners to join our team in Dumfries.
You will provide high-quality, person-centred care and support to individuals in their own homes, helping them maintain their dignity, independence, wellbeing and quality of life.
Key Responsibilities
- Provide person-centred care and support in accordance with individual care plans.
- Assist with personal care, medication, meal preparation and daily living activities.
- Promote independence, dignity, choice and wellbeing.
- Support individuals with appointments, social activities and community participation.
- Provide companionship and emotional support.
- Follow care plans, risk assessments and organisational policies.
- Maintain accurate and up-to-date care records.
- Identify and report safeguarding concerns, incidents or changes in people's needs.
- Work effectively with colleagues, families and other professionals.
- Attend supervision and complete mandatory training.
- Uphold the SSSC Codes of Practice and maintain professional standards.
