Overview
We are seeking an experienced Care Coordinator to join our dedicated team within the senior care sector. The successful candidate will be responsible for organising and overseeing personalised care plans, ensuring high standards of care and support for our residents. This role offers an opportunity to utilise your leadership skills, communication expertise, and knowledge of care home operations to make a meaningful difference in the lives of our residents. A valid driving licence is essential, as the role involves travel between care facilities and community engagements.
Responsibilities
- Develop, implement, and review individualised care plans in collaboration with residents, families, and healthcare professionals
- Supervise and support care staff to ensure delivery of high-quality care in accordance with organisational policies and regulatory standards
- Coordinate appointments, visits, and activities to promote residents’ wellbeing and independence
- Maintain accurate documentation using IT systems and ensure compliance with data protection regulations
- Act as a point of contact for families, healthcare providers, and external agencies to facilitate effective communication and service delivery
- Organise staff rotas and oversee daily operational activities within the care home environment
- Monitor residents’ progress and adapt care plans as necessary to meet changing needs
- Ensure adherence to health and safety protocols within the care setting
Experience
- Proven supervisory or leadership experience within a care home or senior care environment
- Demonstrable experience in developing and managing care plans tailored to individual needs
- Strong communication skills with the ability to liaise effectively with residents, families, healthcare professionals, and team members
- Competent in using IT systems relevant to care management and documentation
- Valid driving licence is essential for travel between sites and community visits
- Experience in supervising staff teams, with a focus on fostering a positive working environment
