Q Care are recruiting a part time Care Coordinator to join their Cardiff team!
This position will be to cover MAT leave.
Purpose of role: Responsible for coordinating the work schedule of support workers in defined geographic areas within Cardiff utilizing the People Planner software. This is to be completed as efficiently and effectively as possible in accordance with current care standards, relevant legislation and Q Cares policies and procedures.
Main Duties and responsibilities:
- To produce a weekly schedule for support workers by 5.00pm every Wednesday for calls that will commence the following Monday this will be in accordance with Q Care guidelines and contractual obligations.
- To ensure client needs are met by scheduling care to be delivered at the times stipulated on the ISC / Service Contract and of accuracy of relevant paperwork.
- Ensure we provide the appropriate service to meet individual needs as per the service users individual care plans and company ‘Your Plan’.
- Ensure all logs for decreases/increases and terminations are maintained on a weekly basis.
- To ensure that calls are scheduled with appropriate travelling times and adhere to legislative requirements according to the Working Time Regulations ensuring sufficient rest breaks. This may mean covering care calls yourself.
- To follow company processes in line with sickness and leave
- To ensure that calls are scheduled in line with staff availability and continuity thresholds
- Achieve high levels of continuity for both service users and staff
- In cases where calls require covering at short notice i.e. sickness or short notice requests to ensure that all care calls are appropriately covered prior to the scheduled time
- Responsible for planning new referrals to assist with growth and ensure that the branch team meet and/or exceed the projected hours targets on a weekly/monthly basis
- Responsible for liaising with brokerage teams, ensuring that responses to referrals and quotes are dealt with in a timely manner as stipulated by the local authority
- Ensure all amendments or queries are dealt with promptly, efficiently and logged on people planner and forwarded to brokerage.
- To ensure that all relevant documentation is passed to the appropriate person in relation to packages of care
- To monitor and action People Planner roster alerts ensuring to refer to appropriate line manager where consistent occurrences arise.
- To action any unconfirmed booking on a daily basis and as deadlines directed by finance
- To ensure that systems are accurate and up to date in order for the Company to effectively process payroll and invoice data
- Communicate and record any changes or problems arising with support workers and service users to the appropriate line manager
- Assist In the management of Q Cares absence monitoring procedures through completing return to work interviews and recording absences to the appropriate persons
