The Role: Dementia Care Manager
Our Dementia Care Managers lead on the care and management of our Dementia Community with the home. They deliver and provide all the dementia related learning to the rest of the team and provide support and knowledge to the residents and their families.
Key Responsibilities:
Residential Care- Understands and promotes the Hallmark Dementia Strategy while working alongside the team to ensure relationship centred care is delivered to a high standard. Plans, develops and organises the overall operation of the Dementia Community.
Compliance- Developed a working knowledge of current and evolving laws of policies and practice while ensuring the dementia community meets all CQC standards and regulations. Assisting leadership to complete any compliance documents.
People Skills- Creates, delivers and monitors team members training. Available to attend annual Dementia Care Managers Conference and work with our Lifestyles team to create appropriate activities that offer joy and meaning to our residents.
Financial Management- Work alongside leadership to utilise the home budget for the dementia community and also understand the costs associated with dementia care.
Family Service- Communicate with families, GP’s and other agencies for care review meetings. Build relationships with the residents and their families to help provide support for them.
About You:

Practical experience of working in dementia care and creating advanced care plans.
- Knowledge of care standards and the drive to keep or meet and ‘outstanding’ CQC level

Empathy- this is such an important role and sensitive issue can arise, clear and concise communication skills are also needed
A high level of understanding how to develop meaningful activities for our residents
