We have an exciting opportunity for an Integrated Support Worker to join the South Falls Team, Midlands Partnership NHS Foundation Trust, in Staffordshire and Stoke on Trent.
We are looking for highly motivated individuals, who are keen to make a difference in this rapidly evolving clinical area. The successful applicant will have the opportunity work alongside the Operational Lead, Clinical Lead and clinical colleagues.
The South Falls Team provide high quality assessment to patients across Staffordshire who present with falls.
You will provide both clinical and administrative support to the South Falls Team following agreed pathways to assist in the implementation of rehabilitation and therapy programmes in addition to review of equipment predominantly provided within patient’s homes and working alongside dynamic clinicians across the Staffordshire area.
