Ribble Recruitment is currently recruiting on behalf of the London Borough of Hackney for an experienced Occupational Therapist to join their Adults Health & Integration service.
This is an excellent opportunity for a qualified Occupational Therapist to support adults with complex and substantial disabilities, helping residents maximise their independence, improve wellbeing, and remain safe within their own homes and communities.
Working as part of Hackney’s integrated neighbourhood model, you will complete comprehensive community-based assessments, provide specialist Occupational Therapy interventions, and work collaboratively with residents, carers, health professionals, and partner agencies to deliver preventative and person-centred support.
The role focuses on identifying solutions that enable residents to maintain independence, reduce reliance on ongoing care services, and achieve improved outcomes through equipment, adaptations, assistive technology, and personalised care planning.
Key Responsibilities
- Complete comprehensive Occupational Therapy assessments for adults with permanent, complex, and substantial disabilities.
- Carry out assessments within community settings, including service users’ homes.
- Develop strengths-based, outcome-focused plans that promote independence, choice, and wellbeing.
- Identify preventative interventions that reduce the need for ongoing care services.
- Assess and recommend specialist equipment, Telecare, assistive technology, and adaptations.
- Review existing care packages and support arrangements to ensure they remain effective.
- Complete functional assessments to support care planning and independence.
- Provide clinical reasoning and evidence-based Occupational Therapy recommendations.
- Work collaboratively within a multidisciplinary team alongside Social Workers, Health professionals, and partner agencies.
- Support moving and handling assessments and promote safe living environments.
- Identify safeguarding concerns and contribute to risk management plans.
- Maintain accurate case records, reports, and electronic documentation.
- Provide supervision and support to Occupational Therapy Assistants and students where required.
About You
The successful candidate will have:
- A recognised Degree in Occupational Therapy.
- Current HCPC registration.
- Experience completing Occupational Therapy assessments within Adult Social Care/community settings.
- Experience supporting adults with physical and sensory disabilities.
- Knowledge of relevant legislation including:
- Care Act 2014
- Mental Capacity Act
- Housing Grants, Construction and Regeneration Act
- Experience recommending:
- Specialist equipment
- Minor adaptations
- Complex environmental adaptations
- Assistive technology
- Strong understanding of preventative approaches and promoting independence.
- Experience working within multidisciplinary teams.
- Excellent communication and report-writing skills.
- Ability to manage a caseload independently.
- Enhanced DBS clearance.
