Overview
We are seeking a dedicated and compassionate Care Coordinator / Senior Support Worker to join our team. The ideal candidate will possess strong leadership qualities, excellent communication skills, and experience in supervising care staff.
A valid UK driving license is essential to this role to facilitate community visits and support coordination. This position offers an opportunity to make a meaningful difference in the lives of vulnerable individuals while working within a supportive and professional setting. You are also expected to cover shifts in the community as a Senior Support Worker, hence, you need to have experience in care at a senior level.
Duties
- Lead, supervise, and support a team of Support Workers
- Oversee the development, implementation, and review of personalised care plans
- Supervise and coordinate team activities during assigned shifts to ensure operational targets are met
- Ensure compliance with company policies, health and safety regulations, and quality standards
- Assist in planning shift schedules and delegate tasks accordingly
- Ensure all care visits are allocated appropriately and delivered in line with client needs
- Assist with client assessments and the development of person-centred care plans
- Monitor and review care packages to ensure high standards of care are maintained
- Communicate effectively with management regarding shift progress, challenges, and staff needs
- Support onboarding and training of new team members as needed
- Assist clients with daily activities such as personal hygiene, dressing, and mobility
- Experience in care coordination, case management, or a related health/social care role
- Flexibility to adapt to changing priorities and shift requirements
- Liaise effectively with external healthcare providers, social workers, and families to ensure comprehensive support

Requirements
- Strong leadership skills with the ability to motivate and guide team members effectively
- Experience within domiciliary care
- Experienced in scheduling within a domiciliary care setting
- Understanding of CQC standards, safeguarding, and care planning
- NVQ Level 3 in Health & Social Care (or willingness to work towards)
- Excellent communication skills, both verbal and written, for liaising with residents, families, and healthcare professionals
- Valid driving license and access to own car are essential for community visits and operational flexibility
- Proficient IT skills for maintaining accurate records and documentation
- Knowledge of safeguarding procedures, health & safety regulations, and compliance standards in adult social care
- Compassionate approach with a genuine desire to improve service users' quality of life
- Ability to work effectively as part of a multidisciplinary team whilst demonstrating organisational skills and attention to detail

Pay: £28,392.00-£31,157.00 per year
